The forms on this webpage will help you get the job done when it comes to helping UF graduate students to keep their academic careers on track.
Please read the instructions for each scenario carefully to ensure accurate and timely submission. Most processing of these forms is completed through GIMS (Graduate Information Management System) or the Graduate School Petition Portal, linked in the instructions.

Applicants or students use this form to signal interest in a fellowship or assistantship from your academic unit. This form goes directly from the applicant or student to your unit.
Academic units use this form to transfer of eligible prior course credits to a UF graduate student’s degree program. You’ll find it on the student’s individual degree segment in GIMS (Graduate Information Management System).
Academic units use this form to set up a program of study involving limited shared credits when a UF graduate student wants to pursue two master’s degrees at the same time. The academic unit submits this form for the student through the Graduate School Petition Portal.
Academic units use this form to set up a student to pursue graduate and professional degree programs at the same time. You’ll find it on the student’s individual degree segment in GIMS (Graduate Information Management System).
Academic units use this form when a UF PhD student wants to pursue, at the same time, a master’s degree outside of that PhD academic unit. You’ll find it on the student’s individual degree segment in GIMS (Graduate Information Management System).
Academic units use this form to request a third and final lifting of a registration hold for a UF student whose non-US academic credentials are still missing at the UF Office of Admissions. Academic units submit this petition for the student through the Graduate School Petition Portal.
Option 1: Academic units can use this form to formally add non-UF individuals as guest experts to graduate students’ supervisory committees. You’ll find it on the student’s individual degree segment in GIMS (Graduate Information Management System).
Option 2: Academic units can choose to informally add non-UF individuals as guest experts to graduate students’ supervisory committees. You can do so internally by direct invitation, with written approval from the student and the supervisory committee chair kept on file in the student’s home academic unit.

This form is for UF graduate students who want to take courses as a higher education institution elsewhere (inside or outside of Florida’s State University System) with the United States and apply those credits to their UF degrees. Students or academic units can initiate this process. Students or their academic units can submit the form to the Graduate School.
For all other scenarios, academic units use the petition form within the Graduate School Petition Portal.
Petitions are requests for an exception to current UF policy due to extenuating circumstances beyond the control of the student, faculty, or staff.
They must come from a graduate coordinator, program director, department chair, school director, or supervisory committee chair in the student’s home academic unit.
They must be approved and signed by the student’s home academic unit and college before submission to the Graduate School.
